Always mix the yeast with flour first, then add salt. Or dissolve the salt in a separate liquid. Keep them from touching until they’re in the dough family.
Is your flour too old or low-protein?
Old flour loses its magic oomph. Also, all-purpose flour works, but bread flour has more protein (gluten), which traps the gas better. Low-protein flour? Your dough will be a flat pillow.
If you’re using whole wheat, add a little extra water. Whole wheat is thirsty like a camel and makes the dough denser.
